Essential Forms

It is essential that you complete and submit the required prior to taking part in any ringette activity . Completed forms must be sent to GCRAFORMS@Gmail.com

All participants under 18:

• Informed Consent & Assumption of Risk - for all athletes under 18, parents should be signing this
• Declaration of Compliance
• Addendum to Declaration of Compliance (for travel to Quebec)

 
All participants over 18:

• Declaration of Compliance
• Addendum to Declaration of Compliance (for travel to Quebec)
• Release of Liability, Waiver of Claims and Indemnity Agreement - All participants over the age of 18 should be signing this
